Named accountable GP

All patients, including children are allocated a named, accountable GP.  If you are unsure who this is then please contact reception who will be able to give you this information.

 

New Patients

To register you will be asked to complete a registration form and a medical history questionnaire. You may be invited to see a practice nurse for a new patient health check. You will be registered with the practice but may express a preference to see your usual doctor.

You may make appointments Monday to Friday by calling 01737 244325 (OPTION 1) from 8.00am to 6.30pm or in person from 8.30am to 6.30pm.

Home Visits

Visits are available to the housebound and those who are too ill to attend the surgery. Please request home visits by calling 01737 244325 (OPTION 1)between 8.00am and 10.30am.

 

Practice Charter

Telling us if you move or change your telephone number. We may need to contact you urgently Attending your appointments on time Informing us as soon as possible if you need to cancel appointments, so they may be allocated to someone else.

 

Disabled Facilities

The surgery has ground floor toilet facilities for the disabled.

 

Carers

If you are a Carer the Practice would like to ensure that you receive as much help and support as you require. There is a Carers Information Pack available at reception containing advice and details of local support services. You can also download a carers registration form from the forms/leaflets tab opposite or contact Tel: 0808 8020202 or www.nhs.uk/carersdirect for further information about carers.

 

 

Services Not Available on the NHS

The NHS pays the Practice to provide General Medical Services. There are a number of activities that Doctors undertake which are not classed as NHS services and as such are private services which have to be paid for by the patient. They include:

  • Private certificates for your employer
  • Fitness to drive examinations
  • LGV and PSV licence medicals
  • Fitness to travel forms
  • Holiday cancellation documents
  • Medical examinations
  • Private medical insurance claim forms
  • Ofsted forms
  • Fit to fly, model, parachute letters
  • To whom it may concern letters
  • Certain travel vaccinations

We do not discriminate on the grounds of disability or medical condition.

 

Our staff will treat you with courtesy and respect. Equally, we ask you to treat them with the same courtesy and respect. Abusive, aggressive or threatening behaviour towards our staff, nurses and doctors is not acceptable and such behaviour will result in patients being removed from our list.

 

The Practice may close occasionally for staff training. Normally this will be for a short period on a Tuesday lunchtime and calls will be answered by the out of hours service.

 

Consideration to other patients

We ask patients not to use mobile telephones inside the surgery and to keep children as quiet as possible because some patients will be feeling very unwell. Also, please do not smoke or eat in the building.

 

The Protection and Use of Patient Information

We ask you for information about yourself in order that you can receive proper care and treatment. All information about our patients is treated as strictly confidential. We only pass on information about you to other Doctors or health professionals who may need the information to treat you.

 

The sharing of some types of very sensitive personal information is strictly controlled by the law. The NHS Central Register for England and Wales contains basic personal details of all patients registered with a GP. It does not contain clinical information.

 

Patient information will only be released to other organisations with the express written permission of the patient. Anyone who receives information from us is also under a legal duty to keep it confidential.

 

The practice is registered in accordance with the Data Protection Act 1998 and the Practice Manager is the designated Data Controller.

 

Under the Data Protection Act 1998 patients have a right of access to their medical records. Details of the procedure are available from reception or by telephone. We are permitted to make a charge for providing access to, or copies of, medical records.
