TERMS AND CONDITIONS OF SERVICE
By making a payment you agree to the terms and conditions as stated below:
Private work Terms and Conditions
· Please note all private work is chargeable and not covered by the NHS
· Private work has an estmated turn around time of 28 working days, on occasion can be longer depending on clinician availability and t6he complexity of the request. Refunds will only be considered if no work has been done, if work has taken place, partial refunds will be at the practice’s discretion. If all work is completed but took beyond 28 working days, refunds will not be given as work has been completed.
· Please note the above time frame starts once payment is received
· Payment will be required in advance
· For private work that needs face to face appointments, there will be a non-refundable full-payment required for the appointment to be booked, subject to other terms noted below.
· Cancellations of face-to-face appointments with one weeks notice where no work has been carried out will be eligible for a partial refund.
· Cancellations of face-to-face appointments with less than one week’s notice but more than 48 hours' notice will lead to 50% of payment being retained by the practice which is non-refundable.
· Cancellations of face-to-face appointments with less than 48 hours notice will lead to full payment being retained by the practice with no refund.
· Rebooking of any face-to-face appointments after a cancellation in line with above terms will only take place once a further payment is received, any payment lost due to late cancellations will not be put towards the cost of the private appointment/report.
· If the surgery has to cancel the face-to-face appointment for unforeseen circumstances, we are not liable for any losses you may incur but will either rebook you if agreeable with both parties or the practice will provide a full refund if the service / appointment is no longer required by the patient.
· If a face-to-face private assessment and report leads to further work due to a finding, extra appointments and further forms may incur a further charge.
· Our pricing for private work has a baseline. Costs may increase depending on if it is required before our 28-day time frame, the complexity of the information required, the time it will take the clinician to complete and the availability of the clinicians to complete this private work. This costing will be at the practice’s discretion at the time.
· Reports / letters will be factual, and the clinician will decide what is appropriate to write, payment does not mean that the clinician will write what the patient is requesting as this may not be appropriate or we may not have the information stated in our notes. If we are provided more information by you (the patient/guardian), we may include this but be clear that this is a statement provided to us, and we cannot confirm it as facts.
· If outcomes from private letters / reports / medicals does not lead to the patient’s desired outcome, for example insurance declined, claims declined, failed taxi medicals (this list is not exhaustive), the practice will not provide refunds. We are not liable for any outcomes/losses. The payment is for the work carried out.
· If a report / medical / assessment leads to further requests for information / further assessments / tests / forms to be filled, there will be further charges, as the original payment would only cover the first part of the work and is not inclusive of any further work that may be required. If any assessment leads to further investigations or assessments that would be normally done via the NHS, then you will be advised as such, booked into the most appropriate appointment and there would not be a charge for this. If this is not to your satisfaction and you wish to cancel the service, then work already completed will not be refunded.
· Please note that complaints related to private work does not fall under the NHS complaints procedure / requirements and would be assessed by the practice separately. You will still address all complaints to the practice manager however the same process will not apply.
· Our private fees are continually being reviewed and subject to change at any time at the practice’s discretion, previous payment for reports does not mean that further reports will be charged at the same cost.
· The practice has the right to decline private work as it is not part of our NHS work and again is at the discretion of the practice.
By making a payment you are agreeing to abide by the terms and conditions of the service
